Vital Records

The purpose of Vital Records is to maintain records of all births and dealth occurring in Sumter County and to assist in the filing and issuance of any such certificates.

Hours:

8:00AM - 5:00PM Monday through Friday

Services Provided:

Issue birth certificates (ID required)
Issue death certificates
Assist in the amending of birth and death certificates
Assist in the filing of a delayed birth certificate
Issue burial permits (funeral homes only)

Paternity acknowledgments

Birth and Death Certificates can be found at Vital Records. To contact Vital Records in Americus, you can call: (229) 924-3637. Either certificate costs $10 for the first certificate and then for each SAME certificate following that it is, $5.

Other Services

Immunizations

Immunizations are provided for early childhood diseases such as DPT (Diphtheria, whooping cough, and tetanus), polio, MMR(mumps, measles, and rubella), Hib (meningitis), HBV (hepatitis B) - is an ongoing service. Adult immunizations are available. Call for more information about other vaccines and requirements.

Health Education

Health Education is provided to interested persons, organizations and agencies. Focus on prevention and education in various health topics. Video loan, AV equipment, educational pamphlets, resource speaker, assistance with program planning, information and referral source.

Women's Health

Women's Health provides a complete physical exam including pap smear and creening for HIV/AIDS, sexually transmitted disease. Also, to include education and counseling on methods of contraception, Birth Control options include barrier methods, pills, Norplants and/or injections.

Healthcheck

Healthcheck is a comprehensive physical exam for children which includes developmental and nutritional assissment. Screening for lead, vision, hearing and dental are a vital part of this clinic. this program is intended for children birth to 21 years of age which are Medicaid/Non-Medicaid recipients. Special emphasis is placed on follow-up of high risk infants.

Genetics

Genetics counseling for families, early detection of birth defects through screening for at risk mothers and children.

Environmental Health

Environmental Health provides preventive health strategies for the community. Several major prpgrams are: food serice inspection, air quality monitoring, on-site sewage system evaluation, injury control, infant and toddler car seat, tourist accommodations, rabies control, and health related complaints.

WIC

WIC provides comprehensive health and nutrition services that encouage wellness, healthy lifestyles and prventive health care. Nutrition education services and nutritious supplement foods are available to eligible Women, Infants and Children up to five years of age.

District Clinical Services

District Clinical Services HIV/AIDS education, counseling, testing, partner notification, surveillance, case management and clinical services are all available by appointment.

Children First

Children First provides a system of identification, linkage, monitoring, access to primary health care and service coordination for children 0-3 years of age who are at risk for poor health and/or development.

BreasTest and More

BreasTest and More screens for breast and cervical cancer, breast exam, pap smear, screening mammogram for women 50 and over, low-income, no insurance or underinsured.

STD Screening and Treatment

STD Screening and Treatment provides confidential screening and treatment of sexually transmitted diseases, HIV counseling, testing, and confidential partner notification.

Car Seat Program

Car Seat Program - provides car seats and training on proper use of car seats. Call for eligibility and class schedule.

Hypertension Services

Hypertension Services provides a hypertension clinic to offer detection, treatment, monitoring, referral, follow-up and patient education in regards to some of the chronic disease such as hypertension.

Laboratory Services

Laboratory Services provides low-cost laboratory services. Call for details and appointment.

Babies Can't Wait

Babies Can't Wait provides health education and social services for families of children who may be developmentally delayed. Birth to three years of age.

Children's Medical Services

Children's Medical Services provides treatment and services to children with special health care needs, also helping them to attain/maintain their optimum level of health and functioning. Children age birth to twenty-one years.
