Position open to current and former State employees and all others who meet the qualifications.
Pay Grade: 12 District Entry/Hiring Salary $26,672.16/year
Maximum Hiring Salary $30,406.32/year
Salary based on education and experience

LOCATION: Babies Can’t Wait, Columbus Health Department

DUTIES: Assembles information, screens referrals and applicants for eligibility, and assigns service priority. Conducts face to face comprehensive assessments to identify problems and service needs. Develops comprehensive care plan. Coordinates the implementation and management of services for consumers and/or families. Must have transportation (personal vehicle and driver’s license). Individual will provide both intake and ongoing case management services.

QUALIFICATIONS: Bachelor’s Degree in social work, psychology, counseling, sociology, education, nursing, vocational rehabilitation, or a related field and two (2) years of full-time (or equivalent part-time) work experience in social services delivery (case management, outreach, prevention/education, etc), OR a Master’s Degree in social work, psychology, counseling, sociology, education, nursing, vocational rehabilitation, or a related field. A copy of college transcript is required.

PREFERRED: At least two (2) years of professional experience providing direct services to children with special health care needs, developmental delays or handicapping conditions (birth to 21) and/or their families. Resume must clearly document percentage of applicant’s caseload in the above experience.
