Vital Records

The purpose of Vital Records is to maintain records of all births and dealth occurring in Muscogee County and to assist in the filing and issuance of any such certificates.

Hours:

8:00AM - 4:30PM Monday through Friday

Services Provide:

Issue birth certificates (ID required)
Issue death certificates
Assist in the amending of birth and death certificates
Assist in the filing of a delayed birth certificate
Issue burial permits (funeral homes only)
Legitimations
Paternity acknowledgments

Birth and Death Certificates can be found at Vital Records. To contact Vital Records in Columbus, you can call: (706) 321-6130. Either certificate costs $10 for the first certificate and then for each SAME certificate following that it is, $5.

If a person was born at a hospital in Columbus, GA at Doctor's Hospital, Medical Center (old City Hospital), St. Francis, or Bush Hospital (now closed), you can contact Columbus Health Department.
